What is the Fagerström Test?
The Fagerström Test for Nicotine Dependence (FTND) is one of the most widely used clinical tools for estimating nicotine dependence. Developed by Karl-Olov Fagerström, it uses six questions to produce a score from 0 to 10.
Healthcare providers use this score to guide quit-smoking support — including whether nicotine replacement therapy (NRT) or other medications may help.
Scoring guide
| Score | Dependence level | | --- | --- | | 0–2 | Low | | 3–4 | Low to moderate | | 5–7 | Moderate | | 8–10 | High |
Higher scores generally indicate stronger physical nicotine dependence and a greater benefit from structured quit support.
The six questions cover
- Time to first cigarette after waking
- Difficulty refraining in forbidden places
- Which cigarette is hardest to give up
- Daily cigarette count
- Morning vs. rest-of-day smoking pattern
- Smoking while ill in bed
Each answer maps to a weighted score defined by the original Fagerström protocol.
